Please send in the completed form as soon as possible and no later than 12 noon the day following the incident.
Incident reports must be submitted to the OBHF by the Referee, NOT just the member league, for anything involving a Game Misconduct and above. The referee can submit the report to the OBHF and the League, but must directly submit it to the OBHF. It is NOT the responsibility of the member league to get the reports to the OBHF.
When filling out these reports, please give us as much information as possible such as the score of the game, events leading up to the incident, player’s behaviour after the incident, etc. It is important that we maintain a strong record of suspensions so that we can keep our game clean, safe and fun for everyone involved. If you plan on filling out the report from home, it is easy to take a picture of the parts of the game sheet with your phone to ensure you are accurate in your reporting.
Major Penalty, Match Penalty, Gross Misconduct and/or Game Misconduct Penalty is assessed during the game.
A written report must be emailed by the referee (not the league) to: discipline@ontarioballhockeyfederation.ca before noon the following day by the referee making the call. This due to the fact that the player may be registered with multiple teams in multiple leagues and could have a game the following evening. We need the time to communicate with the necessary parties.
The updated suspension details will be added to the suspension list (located on the OBHF website under Suspensions) and an email will also be circulated to all member leagues.
The OBHF will apply the recommended suspension guideline to that player unless a reason to do otherwise exists and assume that the team he is suspended with plays 1 game per week. That will mean if the player gets a 4 game suspension, he is suspended from all OBHF activity and events for 4 weeks.
All parties involved with be supplied with the number of games + date of eligible return to eliminate any confusion.
If, the suspended player’s team plays more than 1 game per week with the team in which they received the suspension, it will be on the onus of the player and home league operator to produce evidence of that and then the “date of eligible return” will be modified accordingly.
Any player that makes an attempt to circumvent an OBHF suspension will have their suspension automatically doubled and their status within the OBHF will be reviewed.
The OBHF has implemented a Level-based system which will recognize prior experience at either the Minor or Adult divisions. Payment will still be based on the division you intend to officiate in.
The levels will be as follows:
Level 1 – Officials ‘new’ to ball hockey with 0 to 3 years experience regardless of division (Minor or Adult). Your clinic will consist of an online exam and mandatory on-floor session conducted by an OBHF approved instructor where fundamental positioning, technical procedures, and face-off locations will be the focus of instruction.
Level 2 – Officials with 3-5 years experience regardless of division (Minor or Adult). Your clinic will consist of an online exam and recommended in-person meeting with your local league RIC with an emphasis on OBHF standards and expectations as outlined by the Provincial RIC & Technical Director.
Level 3 – Officials with 5+ years experience regardless of division (Minor or Adult). Your clinic will consist of an online exam and suggested in-person meeting with your local league RIC with an emphasis on OBHF standards and expectations as outlined by the Provincial RIC & Technical Director. This meeting will be at the discretion of your local league operator should it be mandated or voluntary.
Please note that the OBHF has maintained registration histories for 6 years and can verify previous experience based on past registrations. We ask that you be forthcoming and honest in selecting the appropriate registration form to complete.
Ice hockey officials new to ball hockey can apply for Level status by completing the Google Form below. Once confirmed, those officials can complete the appropriate form and begin the registration process.
Once payment has been confirmed by Lino, clinic materials will be emailed to you.

The following incentives and offers will remain in place for the 2025 season.
-Clinic Fee Reductions – The Adult fee remains at $50, and the Minor fee remains at $25
-Family/Household Discount – Officials who are immediate family members, will be responsible for submitting one fee. If both officials are Minor referees, they will pay $25 collectively. If one official is seeking Adult certification, the Adult fee of $50 will be applied.
-Finder’s Fee – Any official who recruits a new/formerly certified official, will receive a $25 discount on their fee whether it be an Adult or Minor registration. (Formerly Certified is defined as an official who was not certified during the 2023 season.)
-Official Start-Up Package – We continue to offer our new and returning officials a discounted price on an ACME Thunderer whistle and sublimated referee jersey at the OBHF’s cost.
